Commissioning Boilers

Following a recent HSA investigation of a hot-water boiler explosion which occurred while the unit was being installed as part of a heating system, the Authority wish to highlight the importance of proper isolation and lock-out procedures when commissioning or maintaining plant. The explosion occurred because a breakdown in communication led to the boiler being fired up whilst isolated from its safety devices. The incident also highlighted the need to ensure that relief valve settings are appropriate for the application, as it was found that the pressure setting on the safety valve was incorrect.

The HSA points out that such boilers in workplaces are classed as work equipment in the context of the Safety, Health and Welfare at Work (General Application) Regulations and are subject to periodic inspection by a competent person and the results of such inspections have to be kept available for inspection for 5 years.  Regular checks also need to be carried out as per recommended maintenance procedures.
